FAILURE MODES AND EFFECTS ANALYSIS
Overview:- FMEA also known as Failure Mode(s) and Effect(s) Analysis, is a
common tool which is used in the Analyze and Improve phases of the
DMAIC
methodology. With FMEA, (high) potential risks of the product and or process
are determined.
FMEA, applied to a design is called a Design FMEA
(DFMEA) and the abbreviation for Process
FMEA is PFMEA. These (high)
risks are based on the effect (impact) and the cause (likelihood that a failure
could occur without detecting it) of a failure.

Failure Mode and Effect Analysis is an analytical technique used by a
product design team as a means to identify, define, and eliminate, to the extent
possible, known or potential failure modes of a product or system.
